our club, you must obtain a Medical Marijuana
ID card from a public health agency. We
accept cards from the following agencies: - Oakland Cannabis Buyers ...Oakland Cannabis Buyers Cooperative
- Marin CountyDepartment of Vital Statistics
Physician's Statement Form
A completed Physician's Statement Form
is required to obtain a Medical Marijuana ID card. Once you are in possesion of a valid ID card
from one of the agencies listed above, you may join the club.
